The Emergency Department Medication Reconciliation Technician (EDMRT) works under the direct supervision of a Clinical Pharmacist. The EDMRT obtains medication history for all patients pending admission from the Emergency Department. The medication history includes prescription and nonprescription home medications. The EDMRT will document each patientâ��s home medications in the required hospital electronic record.
To perform this job successfully, the EDMRT must be trained, reviewed, observed, and approved by completion of a role-specific competency which will be evaluated be written and performance assessment, completion of â��mockâ�� patient interviews, and a review of skill level and communication abilities.
Minimum Education: High School Diploma or equivalent required.
Minimum Experience: No previous experience required. 1 to 2 years of experience as hospital tech preferred.
Required Certifications/Registrations/Licenses: Current South Carolina Pharmacy Technician certification or a current South Carolina Pharmacist Intern license. BLS Certification required.
