The Emergency Department Medication Reconciliation Technician (EDMRT) works under the direct supervision of a Clinical Pharmacist. The EDMRT obtains medication history for all patients pending admission from the Emergency Department.The medication history includes prescription and nonprescription home medications.The EDMRT will document each patientï¿½s home medications in the required hospital electronic record.
To perform this job successfully, the EDMRT must be trained, reviewed, observed, and approved by completion of a role-specific competency which will be evaluated be written and performance assessment, completion of ï¿½mockï¿½ patient interviews, and a review of skill level and communication abilities.
Minimum Education:High School Diploma or equivalent required.
Minimum Experience:No previous experience required.1 to 2 years of experience as hospital tech preferred.
Required Certifications/Registrations/Licenses: Current South CarolinaPharmacy Technician certification or a current South Carolina Pharmacist Intern license.BLS Certification required.
